We are a growing company that is looking for LVN/LPN, RN, LMSW, LCSW and MSW care managers for many areas in and around Indio, CA. This can be a part time position to make some extra money, but can also develop into a full-time opportunity, depends on you! This position requires home visits, and you must have your own transportation. You must be comfortable working with little or no supervision. There is actually no hands-on patient care with this job.

You will be case managing patients for their insurance company, providing much-needed guidance to help keep them from returning to the hospital. You must have at least 1 year of experience, provide a current TB test and be willing to undergo background checks and drug screening. This is a very rewarding job and an excellent skill for you to have on your resume as a nurse.In addition, to in-home visits, we also need telephonic care managers.

Telephonic Care Managers may work from home and can work from a cell phone and or a land line. There are several flexible shifts available.If you are interested in the positions listed above, please respond to this Indeed ad.

Geriatric/ Case Management: 1 year
LVN/LPN must have at least 3 years Care Management Experience
