BiLingual Community Coordinator for Integra Service Connect in Los Angeles, CA

Some responsibilities include:

Essential Duties and Responsibilities:

– Find member populations that are difficult to locate (homeless, severe mental illness, substance use, medically underserved, in need of preventive services, etc)
– Schedules Nurse Practitioner Home visits with member
– Updates and confirms appointment information including: date, time, location, NP name, reason for visits
– Informs member of necessary items to have available (including identification card, insurance card, medications
– Informs Member of appointment expectations and guidelines to avoid cancelations
– Work with members to provide effective and efficient service coordination
-Provide on-site/in-home member assessments for safety risk, health needs and barriers to care
– Engage members to achieve health management goals through the use of health coaching, motivational interviewing, and problem solving techniques
– Accurate, Quality, Timely and consistent documentation in company database of member activities and interventions
– Will be responsible for achieving set goals/KPI’s
– Maintains performance expectations such as: Average weekly appointments kept, Average contacts per day, and other performance metric requirements.
– Punctuality and Attendance
– Other duties as assigned

Candidate Must Possess the Following:

– A Valid drivers license
– Access to an insured car

Education & Work Experience:

– Minimum of High School Diploma required
– Minimum of 1 years of experience working in health, social or community services required
– Minimum of 2 years customer service, sales experience desired
– State-based certification may be required
